Recertification Submission Format

Guidelines:

Once you have at least 500 credits to be scored, you will need to submit a printed copy of your Recertification Applicationpdf and CDs containing all of your information to the ABVP management office. All text files on the CDs must be in Word format (.doc file extension). Remember, please print a copy of your application to be filled out and submitted, do not include an electronic copy on your CDs. 

The deadline is January 15 of each year. CD labels are available (call 800-697-3583 or email abvp@xmi-amc.com for labels) -- OR you may choose to create your own. If you choose to create your own labels, please use the following format:

Colored labels must have your ID # ONLY

No names (ID only), and must be attached to each submitted cd. All elements for recertification must be included for grading.

Incomplete packages will be returned

No exceptions!

 

Use standard labels sized at 1" x 25/8"

Blue labels- 2 total cds

Each cd copy has curriculum vitae, continuing ed, publications, lectures and/or submissions reviewed.

Yellow labels - 4 total cds

Each cd copy contains one or two case reports. Circle the number of case reports you are submitting.

Pink labels - 4 total cds

Each cd copy contains all of your exam items and scanned references

Each cd copy contains your interpretive summaries

Remember - You may use the Record of Recertificationpdf to document credits as you accumulate them. Do not submit your Record of Recertification. Keep it with your records.
